I have an untested fix for the first test.
The second test is actually wrong. According to the code, the following will
create an element that uses the default namespace.
OMElement nameElem = fac.createOMElement("name", null);
I am adding a few more tests to Ruchith's test to cover this case and the case
of an actual unqualified element.
