Yes, IstantCommons is great, but I wouldn't call it "really usable". We 
use it on http://www.wikimedia.it and we often have random errors like 
http://www.wikimedia.it/index.php/File:Failed_to_open_stream.png , 
especially with big images like those added here 
http://www.wikimedia.it/index.php/5_anni,_5_milioni_di_file_multimediali_su_Wikimedia_Commons
 
(obviously this is more frequent when the image scalers are busy; and 
looks like it's happening less frequently, lately, but I'm not sure).
