I said it was really usable, I didn't say it was perfectly functional ;-) We use it on rationalwiki.org. Occasionally there are glitches where the local MediaWiki fails to correctly retrieve a file from Commons and the failure is cached for a while (up to an hour). But in general, it works well enough that we don't bother copying images to the local server at all.
