Lots of ideas...
1) dirty floppy drive
2) something's wrong with your boot disks - you didn't use the high-density
version, did you?
3) hardware problem (unlikely)
4) check internal cables to make sure they're all seated properly (also a
long shot but worth checking if nothing else does the trick)
