The others have offered excellent suggestions. If, after following up on those,
you still have problems, here's another thing to check out. There are EMI noise
filters on the lines to/from the floppy drive. They have a relatively high
failure rate, and can cause a variety of weird symptoms, including the one you
describe. I once had an SE/30 that randomly insisted that I had a floppy disk
inserted in the drive ("this disk is unreadable"), even when I didn't. Replacing
the RC filter pack solved that problem. See
