The paper label PN594-TP in the photo is the giveaway that this is a Farallon card. A Google search will turn up loads of links to confirm this. Eg: http://www.fas.harvard.edu/network/ether.mac.html

The "usual suspects" might want to make a mental note that Farallon part numbers always start, imaginatively enough, with PN.
