Lowendmac.com has, somewhere on it's site, a hard drive Formatter made by FWB. This has always been a pretty good tool. In all cases, you must try many format utilities to find one that works. I roped one down by using Lido on it, turning off the power part way through the format, and restarting. The Apple formatter then took over and finished the job after the message "this disk is not initialized".... I don't recommend this, but it worked on a stubborn drive for me.
