Hi Eric, I might be able to help here......I purchased a shrink-wrapped "Macwrite" in the box off Ebay last year. Naturally, I couldn't resist and inside it has....
Packing list - (code 030-3091-D) dated-7/8/87 1/ 2 disc's - 1 -400k, 1- 800k (code 690-5024 and 690-5087) 2/ Macwrite ring bind manual - still shrinkwrapped! with tell Apple card inside (code 030-0688 and 030-1456) 3/ 8 page booklet titled Macwrite Version 4.6 update (code 031-0524) 4/ 1 page - Addendum Macwrite Version 4.6 (code 031-0245) 5/ Software registration card (code 030-3150) There is a little sticker on the front of the box which says - "Now supports Macintosh SE, Macintosh ii and AppleShare" Will send photos to you if you want (by the way, the link you posted of how the disk should look -no, both have a different look!) I have a "very" original 512k that I can pop the 400k disc into to see what comes up! - if you want.
