When things get too bad and programs start to get damaged and not work, i usualy just install a new fresh copy of windows, but i hated doing that not beacause the installation of windows it self, its what comes after that, the huge pile of programs i need and drivers and stuff, but some times its the only option. You can also try avast's boot time scan, i like that option because some viruses affect the anitviruses' performance since if they auto start with windows, or infect some system files or files being used that can't be treated with windows on, so a boot time scan might do the trick. But first i suggest that you try to back up all your data before you do anything, to protect them from being lost or getting infected.
