To be rationed requires that there be a shortage of supply. There is no shortage of supply for those able to pay - if you can afford the procedure you will get the procedure in the US (organ donations being the exception where there is a shortage and the supply of which by law can not be increased by willingness to pay) - that is not rationing. We do have many who can not afford the current standard of care, 50 years ago we had massively more who could not afford todays standard of care, 100 years ago it was largely unimanaginable.

I do not understand the idea that every improvement, no matter how expensive, must be affordable by all, and if not some injustice has occurred.
