After a long trawl back it seems that this was due to an error in phpki and was
fixed in:
phpki-0.82.13
I can't find the original bug for this.

- Replace md5 with sha1 for signing
Note that the phpki fork here uses
default_md = sha512

We probably ought to do as suggested in
https://bugs.contribs.org/show_bug.cgi?id=8685 and use the fork.
