Above the proposed incorporator is a typo "The name and address the 
incorporator is"

We could probably dump the 'religious' part. I don't recall what the 
instructions say on that anymore.

If you(Mark) have a few minutes, call the Ushicon folks.  Apparently 
they put together an LLC - I don't know how you do that for 
not-for-profit, but it might be a better way to go.

Other than that, everything looks fine.
