OK, there are some clues when starting from the command line.

Firstly, on opening a file I get...

GET CONFIGURATION failed with SK=5h/ASC=20h/ASCQ=00h
totem: simple.c:1462: snd_mixer_selem_get_playback_volume: Assertion
`elem->type == SND_MIXER_ELEM_SIMPLE' failed.

Then when I try to close the application I get...
audio_alsa_out: Resume call failed err=-77


So it would appear to be an issue with the sound system. I am using an
M-Audio Audiphile soundcard. This is a bit of a hi-end tweaky card that
needs a special Alsa utility as a mixer called "envy24control". It looks
like Totem may be making some assumptions other applications don't make
about the sound card.

When I press the Close button it starts up anyway and works. Often I can not
close the application. It just goes to a black screen and becomes
non-responsive. I have to use XKill to stop it.
