I had this problem because at one the past I had manually created a
directory /etc/init.d, and at another time /etc/init.d was a symbolic link
to /etc/rc.d/init.d.
To solve it, I deleted my directory /etc/init.d (after making sure it had
nothing in it) and then installed the latest initscripts (using --force if
necessary). initscripts then did its own thing with /etc/init.d that it is
very happy with.
