1) use real 700M blanks - do not try overwriting 650M (they will say they burned 
right, but they didn't)

2) use a relatively new CD burner (if it burns less than 4x, forget it)

3) use cdrecord -v (that will warn you if it detects a problem with burning the CDs)

4) use a very good CD drive to read the CDs. CD1 was sized at very close to the limits 
of any drive to read.

5) Win XP's built-in burn app is useless - it calculates 700M as 700 * 1000 * 1000 and 
adjusts offsets the same way. TOC ends up being wrong.
( correct math - 700M = 700 * 1024 *1024 )
