I tried the following and the modem is now working: 
 
- deinstall the adi modem rpm. 
- remove stray configuration entries. 
- compile and install a new kernel without the module support for "adiusbadsl". 
- compile and install from tarball from http://eagle-usb.fr.st/ 
 
I'm not sure what the problem was, but I did notice that /etc/hotplug/usb.usermap was 
installed to /usr/lib/hotplug/usb.usermap with the rpm (not sure if that is relevant 
or not).

This happened during the installation and subsequently, whether the modem is
plugged in at power on or whether it is plugged in after the pc has finished
booting.
