My suspicion is that this recipe was originally created by someone who was looking for something extra in the recipe's effects, and that "herbal medicine" they specified, finely chopped, wouldn't be available without a prescription, and then only in certain states. I suspect that any "medicine" in a recipe like this wasn't in there for flavor and can be omitted without too much ill effects.
I hope this is enough of a hint for those of you interested in this little detail without me having to spell it out, as I would prefer not to.
