This is what I discovered:
The authdaemond modules were "tried" in the order of (1) authpgsql and then (2)
authpam and so on.
According to http://www.courier-mta.org/authlib/?README.authdebug.html,
authpgsql should try to authenticate and issue an ACCEPT, REJECT, or TEMPFAIL.
Due to the my poorly configured authpgsqlrc config file, an "authentication
error: Input/output error" would be generated thereby terminating the remaining
queue of authentication modules. I believe this is why No ACCEPT, REJECT, or
TEMPFAIL was issued.
My simple work around was to list authpam before authpgsql in the
"authmodulelist" of the authdaemonrc config file. authpam works fine. So
while imap/pop3 are working fine with authpam, I will have time to read up on
the proper configuration of the authpgsqlrc config file.
