telnet 127.0.0.1 143
a login someusername somepassword
a examine inbox
a logout

Is the delay (a) between connection accepted and login banner displayed,
(b) between login command and success response, (c) after issuing the
'examine' command?

If the delay is at 'a examine inbox' then try:

# su username
$ cd /path/to/homedir
$ strace imapd ./Maildir
a examine inbox

which will show you what syscalls the server is trying to do.
Alternatively, connect (using telnet 127.0.0.1 143), use 'ps' to find out
the pid of the imaplogin process which you've connected to, then use

# strace -p <pid>

to connect strace to it, and watch its syscall operation.

If the delay exists for imap but not for pop3 (port 110), then possibly your
fam or gamin installation is hosed, which is a common problem for
RedHat-like systems.

HTH,

Brian.
