It sounds like things are working except for the "deleted items" folder, so perhaps you just need to make sure the "deleted items" folder has the same owner/permissions as the rest of the Maildir directores. But I don't use Outlook, so I'm just making wild guesses here. FWIW, "deleted items" isn't a default Courier folder, so maybe the problem is that no folder called "deleted items" exists on the server.
