It is completely up to the client how it arranges mail folders visually. The server has absolutely no control over the client's display.
Is anyone using Evolution or Mozilla with Courier-IMAP server?
And are you seeing the INBOX Subfolders as primary sibfolders (not under INBOX, but under the name of the connection)?

PS: I'm unable to see the subfolders of INBOX as primary subfolders, nor am I able to create primary folders (only if I select subfolders of INBOX...)
