The problem has been this (in the event someone is still able to see the problem & offer up a resolution), I send a single page fax ([EMAIL PROTECTED]) from sqWebMail via a USR Courier Ext. V.Everything modem to a Windows NT 4.5 Small Business Server (SBS) equipped w/ USR Courier modems & also to normal fax machines. The SBS never produces a fax file, where as, the normal fax machine will produce a page. In ALL cases /var/log/messages reports a failed transmission of f0000-0001.g3. The SBS, surprisingly, is fairly reliable & has been in service for a year or more.
If I understand you correct, normal fax machines receive and print faxes correctly, while the NT box fails.
Sounds like a problem with the NT box.

Mar 2 10:13:24 sniper sendfax[17212]: failed transmitting
/var/spool/courier/faxtmp/f0000-0001.g3: phone="3409257", +FHS:00,
dev=ttyS0, time=46s, acct="" <snip>
I cp'ed the .g3 before it's deletion & manually ran faxspool 3409257 f0000-0001.g3. It faxes successfully to both the SBS & normal fax machines.
See what happens if you run sendfax instead of faxspool.
sendfax is invoked by Courier as:
sendfax -v -r [phonenumber] filename filename...
Remember as I stated earlier, sending the same file via faxspool & sendfax
manually bypassing courierfax is successful!
All courierfax does is invoke sendfax, as per above.
