What version of courier-imap? Which platform are you running it under? What is the total size (in MB) of messages in the maildir? Recent versions of courier-imap keep a courierpop3dsizelist file which caches the file sizes, whilst older versions had to read every message in the mailbox every time you connected. It could also be worth eliminating Outlook from the equation, by telnetting to port 110, logging in with USER and PASS, and timing how long before you get the +OK back.
