Where is the faq? The only one I found was at the website for courier, and it old and only has a handful of questions. I didn't see any mention of what you spoke of (though I did what you suggested and it didn't help).

www.courier-mta.org/FAQ.html. It's definately being updated often. There is fresh info there every build that warrants it.
As for your other query, verify that authuserdb is declared in the authmodulelist in authdaemonrc. The default specifies:
authmodulelist="authcustom authcram authuserdb authldap authmysql authpam"
If you're still struggling, show us some error logs.
