I just tested with the same settings you have, I am running the latest build of thunderbird. It works fine for me with your configuration #2 I can see the folders properly and I select each of them.
Yep, I just tested Thunderbird 0.1 and the 2003-07-23 release, and both worked fine. And after a little investigation, it seems that a report has already been filed in bugzilla:

Anyway, the bug still exists in 1.5a, but it appears to be fixed in the latest nightly (I just tested, and it seems to work fine). So I guess Mozilla Mail will work properly again once 1.5 is released.
