empty file wont work as desired, from dot-courier(5):

If the .courier file is zero bytes long, it means that default mail deliv-
ery instructions set by the system administrator should be used. If the
file is not a zero length file,  and  does  not  specify  any  delivery
instructions, messages to the corresponding E-mail address are silently
discarded.

but I'd just use |/bin/true for 'mail' in authmysql or authuserdb.
