This is when it got ugly. After that point, any new email to the Inbox doesn't show up in Outlook. Not just this user's Outlook... ANY Outlook. I've tried Outlook 2000 and OutlookXP. I tried deleting the courierimapuiddb file. No dice.
Also as a side note, this Inbox contains 18,000 emails.
Have you ever considered the possibility that Outlook simply shits all over itself any time it sees a folder with EIGHTEEN BLEEPING THOUSAND messages???
Here's a simple test to try:

If you pay close attention to this output, you will see the server reporting 21 messages in this INBOX.
Repeating the same sequence of commands using your own server will show how many messages the server is reporting in your INBOX.
Once you've verified that the server is reporting X messages, and Outlook now shows Y messages, where Y<X, you may proceed to filing a bug report with Microsoft, then holding your breath until it gets fixed.
