This is most often the case when a remote client sends the SMTP QUIT command, but then immediately disconnects without waiting for the session to complete. qmail, postfix and exim all do that, as a way to shave off a few milliseconds of each outbound SMTP connection, since the QUIT doesn't really serve any useful purpose. Courier gets upset about this premature termination, and complains by logging the event.
