From watching the IMAP conversation, Outlook is "getting it wrong" (no surprise there then). It makes no NAMESPACE call, instead relying on LIST "." "*" to find all the folders, and LSUB "." "*" to find the subscribed folders.

But that's just hearsay. You should try the appropriate microsoft.* Usenet newsgroup.

and behave accordingly; or more likely, configure Courier IMAP to use a undifferentiated namespace?

Actually you did miss something very obvious: it is not in Microsoft's interests to deliver robust IMAP support in Outlook; they would prefer you to use Outlook exclusively with Exchange.
