Ok, I switched over to couriertls.  However, this lead me to the real problem.  It wasn't having issues going through stunnel, it only hangs when talking localhost to localhost.  Since stunnel relays and routes from localhost to the localhost server, it had problems.  The reason I know this is a localhost routing problem is because my webmail which uses standard imap still hangs with attachments.  Strange.  I checked routing and iptables, but nothing is getting blocked.

This hang up even occurs when talkin from external ip to external ip and localhost to localhost....ie 1.2.3.4 -> 1.2.3.4 or 127.0.0.1 -> 127.0.0.1.

Is there any reason it would hang on these connections after sending a few hundreds bytes?

Courier fully supports STARTTLS and SSL for POP3, IMAP and ESMTP protocols
and has for some time.  I would recommend upgrading to the latest 0.48.1
version and reading the Documentation at http://www.courier-mta.org
regarding configuring the protocols and SSL.  If you are having trouble
getting these services to work I suggest posting further details like your
server platform and Courier version.
