If not using webadmin, I'd advise using different files for each type 
of entry. E.g., for each relevant IP,

echo '216.239.32.1      allow,BLOCK=' >> smtpaccess/google-whitelist

Comments explaining the case may be added manually when the file is 
created. Note that the default file will likely be overwritten on 
upgrading courier.

Agreed: It is not a good solution for google. However, it is a good 
idea to whitelist any customers you may have that use their own 
sending MTAs, otherwise they cannot email you asking for advice when 
they'll get blacklisted.
