Concerning this, what's the difference about being a small ISP or a bigger 
ISP?

If you want, I *am* a small ISP and our TLS works from the beginning where we 
set up courier (and did so with QMail before).


Our esmtproutes file contains 6 entries about disabling TLS manually for one 
domain. One of them is Deutsche Telekom, the biggest ISP in Germany. The 
others are propably dedicated-servers that are not professionally 
administrated.
I think it's okay to have 6 manual entries in about 4 years of operation.
