What is the best way to create $HOME/Maildir/ for a user before I  
start sorting all their old mbox files into the new folder?

Couriers Maildirs are a little different than the Maildirs that, for  
example, maildirmake or procmail create.  That is to say, at least the  
top level mailbox contains three 'extra' folders:

courierimaphieracl
courierimapkeywords
courierimapuiddb

When are these folders created, and how?  Because until these folders  
exist, I am unable to access $HOME/Maildir with a MUA.

When is courierimapheiracl populate, for instance?  I can create the  
folder, empty, and it seems to stay empty for some period of time,  
then it gets populated with a list of 'top-level' mailboxes

(so, for example, 'courier-users' will appear, but not 'courier-users. 
2009-02' but I've not figured out WHEN that folder becomes populated.   
Is it because I am accessing Courier with a MUA that does not support  
subscribe/unsubscribe (Apple's Mail.app)?  i ask this because the one  
account that I have setup in postbox as well DOES have that folder  
populated.
