Based on this, someone who knows nothing about the inner workings of these authentication methods (i.e., me), would conclude that they should follow the example given and use LOGIN and CRAM-MD5. There should probably be a note here saying that additional configuration may be needed in order to use CRAM-MD5 authentication.

This is a valid point. This language will be clarified.
