Thank you for sharing this. Our current situation is - 
We've been open for 4.5 years and when we first started out we had a few 
issues, talked to the city (code office maybe?) they gave verbal permission 
to have 45 suite numbers, the post office complied with that and we've been 
getting mail for our members with different suite numbers for 4+ years. A 
couple weeks ago we started having people say their mail was being 
returned, address not found or something. 
We talked to the mail carrier, then the post office. New manager just came 
in and some how they don't even have record of ANY of our suite numbers, 
even our main business one! Not to mention the 45 others...
No resolution yet as I'm still trying to figure out how to proceed. I don't 
remember filling out one of the CMRA forms when we started. I think like 
some of you mentioned, the post office may not be familiar with its own 
forms.. very frustrating.
The immediate resolution is that they will continue to deliver our mail 
until April 1 while I figure out what to do.

I guess I should fill out the CMRA form? They were telling my office 
manager we have to get each suite number a permit with the city or some 
craziness?! Any additional advice?
Should I just fill out the form and take it down and see what they say? 
That's probably what I'll do but I'm just nervous. Offering a suite 
number/mailing address is a pretty integral part of our business. Most of 
our members get their business mail here so it's a big deal.
