I looked at mod_perl for a "solution", which is the logical inverse of
what I described below, and that's what I put into 0.16:

#ifndef na
#define na PL_na
#endif
#ifndef sv_undef
#define sv_undef PL_sv_undef
#endif

Not that I'm terribly happy with this, but it does appear to work
correctly.

Thanks for the pointer to Devel::PPPort.  It looks like a more
comprehensive solution, actually, so I may switch in a future version.
