OK good -- the only things I'll change are:
(1) a clean-up for "make warn"
(2) make a "last-pmd-read" cache in ia64_xen_kdump_p2m()
to avoid making as many redundant dumpfile reads
(3) set xkd->p2m_frames to the number of entries in the
p2m_mfn_frame_list page so that they can be dumped
by "help -n".
