I'm not sure if the problem is solved (I cannot read that huge block of
text I've got no-line-break dyslexia or something) but I had a problem like
this on my TF2 server.
It turns out the issue was with me having official maps in the /custom
folder. For example I'd have an (now old) version of cp_coldfront and when
the server moves to that map, everyone is dropped because they have the new
version, but the server is using the one found in /tf/custom/*/maps/
Again this is for TF2 and may not work the same in CSGO but it's another
avenue to explore.
