INFORMATION FOR SHIPMENT OF BLOOD SAMPLES
Blood (10 cc) should be collected in purple-top (contains EDTA)
tubes, mixed and quickly cooled with wet ice and then frozen with DRY
ICE. The blood tubes should be labeled (patient's full name, date of
birth, and date), sealed in a plastic zip-lock bag and placed in a
styrofoam insulated container together with DRY ICE (enough to last
during the shipment).
At the same time, seal the Order Form in a separate plastic bag and
place it in the container together with the blood sample. The
Informed Consent Form, Illness Survey Form and your check (or credit
card information) can be sent separately by regular mail.
Your name, return address and your telephone number should be shown
on the outside of the container.
Ship the container by overnight express courier (such as Federal
Express) for next day delivery to:

Please make sure it will arrive between Monday and Friday.
Foreign Shipment: In additional to the shipment form, you need to
fill out a customs declaration. It might be on the same form as a
separate part. This declaration has to contain the following
information: "Human blood sample for diagnostics. Non-infectious!"
in the field about the contents.
If this information is missing, U.S. customs will check the box with
a delay of at least 2 days. After that time, we are unable to detect
the mycoplasmas in the blood samples! For a fast customs check, a
correctly labeled box is required. You can use the label below in
addition to the customs declaration.
