Getting closer. I removed everything in the ./extensions folder:
$ rm -rf ~/Library/Application Support/Firefox/Profiles/default.rkt/extensions/*
That took care of the problem. Opening/closing windows works, the help menu is back, and I still have my original profile with bookmarks and everything. Something in the ./extensions folders really screwed things up.
