IIRC, you are trying to use this card under a 2.4 Linux kernel.   
Unfortunately, the Open Source driver was designed for the 2.6  
kernel, although the author says it may work with 2.4.  As for  
ndiswrapper, it looks as though you need 2.6 or a later 2.4 kernel,  
according to the ndsiwrapper install page:

"You need a recent kernel at least 2.6.6 or 2.4.26 with source"

 From what you've written before you currently have 2.4.9 on CentOS.   
You probably want to upgrade your kernel, if possible, and then use  
ndiswrapper.

One thing we should have done and didn't because we ran out of time  
was to run Knoppix on your machine and try the card with  
ndiswrapper.  It worked on my machine so it should have worked on  
yours.  Doing that would demonstrate whether or not the card will  
work on your machine.

Tony, what are you doing this weekend?  Wanna meet up at a Bread Co  
for an hour or so to see if we can hash this out?  I'm game on both  
Saturday and Sunday afternoon.
