Click "Keep".
The only stuff setup.exe uninstalls without your explicit consent is stuff
that it's about to install a newer version of - that's how it upgrades stuff,
so it does at least have your implicit consent there.
The default behaviour of setup.exe is to upgrade any existing installed
package any time it finds a newer version in the mirror you're installing
from. The "Keep" option tells setup.exe to keep your existing versions of
everything even if newer ones are available, and you can then select just the
upgrades (or new packages) that you want.
I can't speak to any packaging differences or version conflicts between
-ports packages and official distro versions. It's certainly possible but I
have no knowledge about it one way or the other.
