I have noticed that e.g. a USB HDD can get connected at a different bus:device 
combination after reconnect. Could that play a role? 
Afaik, there is in principle nothing that uniquely identifies an individual 
device, the manufacturer:device IDs only specify the model (HDDs can have a 
label of course, but is that label unique?).
