2. Should it be at midnight UTC? I can't find anything that says one way or the other but I can't imagine that I'm supposed to adjust my clock by 1 second at 11am rather than midnight.

Yes, it is midnight UTC, so that all places around the world have the leap second at the exact same moment. Otherwise places would end up at offsets like -1:59:59 UTC until they had their own local leap second at midnight.

DateTime::Format::Duration is back on track (after upgrading DateTime.pm once I noticed subtractions were broke) and should be released in the next couple of days. Basically I'm making the normalisation a LOT faster.
