45GB - it should take a while :-) Can you take a backup of your production db, put the backup to another machine an run optimize table on dbmail_mimepart there? Just to see what happens. Maybe after optimizing mimeparts, headervalues and messageblks your db size will correspond to the mail size stored in the db.
