I'm lost. In your initial report you said that neither the console nor X change with even a reboot, and that you had to use setxkbmap to make it work in X. But the setxkbmap command you provided was not the same as what is in /etc/default/keyboard, so that's why I'm unsure what you are testing exactly. Did you try to use setxkbmap -rules evdev -model evdev -layout us,us -variant intl,brz -option grp:alts_toggle and see whether it brings you the behavior you expect?
