To reproduce:
From the main menu, select "Execute a shell"
Hit <esc> to return to the main menu
This drops me at a shell, instead of returning me to the main menu.
Caveats:
I hit this testing a d-i TYPE=netboot image on ia64 over serial console.
I'm using ESC as my "go back" method, because selecting the "<Go Back>"
menu item doesn't work.
