I was able to apt-get src courier-mta and run grep against it. I wasn't sure which file exactly was responsible for having an extra 127.0.0.1. My solution was to mv /etc/courier /etc/courier.orig, install everything and then I simply copied the older smtpaccess/default file. I did not modify it. So I'm not sure if this file is to blame.
So, can you diff -urN /etc/courier /etc/courier.orig, and look for 127.0.0.1? Maybe that turns out the culprit.
