I have put the three files into the /musicindex/ directory, as per
the README instructions, and verified that apache2 can read them.
I have restarted apache2 and reloaded the page, yet the images still
come from external sources.
Version of mod-musicindex you're running?
ls -l <musicindex data directory> please?
Even better: browse any directory handled by musicindex with your web
browser. Display the source of the page, and copy paste the lines
comprised between "<!-- begin footer --> and <!-- end footer -->
