Debian does not use an icon cache for /usr/share/icons; either you
 build software locally, or you installed Ubuntu packages, or there's a
 bug in a Debian package.

I did not install Ubuntu packages. All my GNOME packages are from
Debian, mostly from experimental. However, it looks like several
package do call gtk-update-icon-cache from postinst, including
gnome-themes, gnome-themes-extras, gnome-accessibility-themes and
gtk2-engines-spherecrystal.

So if this is an official policy, some developers do not adhere to it.
